Diabetes patient information pack

The information sheets below form a patient information pack that aims to give people living with diabetes an understanding of what care they should expect to receive and to signpost where and how they can receive this locally.  It should initially be given to people:

 

  • newly diagnosed with diabetes
  • with an existing diabetes diagnosis, but who have recently moved to Lambeth or Southwark.

 

The Diabetes Modernisation Initiative (DMI) is evaluating the information packs. If you are a health professional, please include an evaluation form when giving out the pack. If you are a patient downloading the packs directly from this website, please help us improve our services by completing the evaluation form (Word 110Kb).

 

1 - How and where will my diabetes care be provided in Lambeth and Southwark?

Description:
Explains the care you can expect when you have diabetes, including the 15 healthcare essentials defined by Diabetes UK.

2 - Your diabetes - how to take better control

Description:
Gives information about DESMOND, which stands for diabetes education self management for ongoing and newly diagnosed - a free course to help you learn more about type 2 diabetes.

3 - do you have Type 1 diabetes? DAFNE may be able to help you

Description:
Provides information about DAFNE, which stands for dose adjustment for normal eating and is a way of managing Type 1 diabetes for adults.

4 - Other support programmes

Description:
Outlines a number of other programmes available to help you learn the skills to manage your diabetes.

5 - Healthy eating and diabetes

Description:
Explains how a healthy and balanced diet can help you achieve and maintain the best possible control of blood glucose (sugar), cholesterol (blood fat) and blood pressure, to reduce your risk of complications in the future.

6 - Diabetic eye complication screening

Description:
Fact sheet and self-referral information sheet for retinopathy screening for people with diabetes in Lambeth, Southwark and Lewisham, explaining why screening is important.

7 - Feet

Description:
Gives information about how to look after your feet and keep them healthy, including where you can receive routine foot treatment in Southwark and Lambeth.

8 - Would you like to know more about your medicines?

Description:
Information sheet for patients to use with their pharmacist, GP, practice nurse or specialist team to answer questions about medications.

9 - Get involved in your care

Description:
Explains the Lambeth and Southwark Diabetes Patient Forum and local involvement networks (LINKs), which you can join to help influence or change the way diabetes care is delivered locally.
