DESMOND Southwark

In Southwark, the DESMOND course is held monthly and is delivered to groups of up to 15 participants. The course normally runs across two days (over two three-hour sessions). Currently sessions are held on the 2nd and 3rd Friday of each month, from 9.45 am to 12.45 pm. The course is offered to people who have been diagnosed with type 2 diabetes in the last year, although participants may decide to invite a family member of friend to do the course with them.

How to join the course

If you are interested in joining the DESMOND course in Southwark, please contact your practice nurse or GP to refer you. They will need to complete a referral form, providing your contact details, and test results such as blood pressure, HbA1c, total cholesterol and waist measurement.

How to refer a patient

If you are a healthcare professional, you can refer patients to the DESMOND course in Southwark by completing the DESMOND Southwark referral form (in pdf format). The referral form should then be faxed to the DESMOND Southwark administrator:
